FINAL FORMS

The Fremont Athletic Department has partnered with FinalForms to allow you to complete and sign athletic participation forms for your students. FinalForms saves data from season-to-season and year-to-year, so you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

ALL parents of athletes must use FinalForms. Please note; once you register, you will receive an email to confirm your account, create a password, and then complete the required forms.

Along with completing FinalForms, a PHYSICAL copy of the IHSAA PPE Physical (for Physicians) needs to be completed and turned into the school office. The physical must have taken place and be dated after April 1, 2024 to be valid.
Only the IHSAA physical form will be accepted.
